This is a daunting task - there is a good book called Realizing the
Promise of Performance Management that focuses on the key areas that
need to be in place for any performance evaluation process to work.
It really starts with goals so the employees know what is expected
and feedback along the way so that when they do use the formal
evaluation form they should already know where they stand. There
are other books as well that will show standard performance
evaluation forms but the most important element here is how you
manage the Change Management aspect of this. Would be happy to
